ABSTRACT

RESUMO Objetivo Analisar a nota de passagem no perfil de extensão vocal de coristas, pela identificação das frequências fundamentais e intensidades, tanto na mudança para o registro agudo quanto para o grave, comparando-as, por naipe. Método Participaram 67 coristas, com média de idade de 27,79 (±7,50) anos, dos naipes: soprano (n=20), contralto (n=17), tenor (n=15) e baixo (n=15). Para coleta e análise dos dados utilizou-se o software Vocalgrama (CTS Informática) no qual foi realizada a gravação da emissão da vogal /Ɛ/ em glissando ascendente e descendente, até às frequências mais grave e mais aguda, nas intensidades mais fraca e mais forte possíveis. Resultados Foram identificados os valores das frequências e respectivas intensidades das notas de passagem nas emissões ascendentes e descendentes fortes e fracas nos quatro naipes. Houve maior ocorrência de identificação de quebra nas vozes agudas, comparativamente às mais graves. Os valores médios das frequências encontradas foram correspondentes a notas diferentes das estabelecidas na literatura para todos os naipes. Conclusão As notas de passagem identificadas no perfil de extensão vocal de coristas, a partir de suas frequências e intensidades, obtiveram maior ocorrência nos naipes soprano e tenor, comparativamente ao contralto e baixo, nas mudanças para os registros graves e agudos.


ABSTRACT Purpose To analyze the passaggio in the voice range profile of choristers, by identifying the fundamental frequencies and intensities, both in the change to the high and low registers, comparing them, by voice types. Method 67 choristers participated, mean age of 27.79 (± 7.50) years old, of the following voice types: soprano (n = 20), alto (n = 17), tenor (n = 15) and bass (n = 15). For data collection and analysis, the Vocalgram software (CTS Informática) was used, which recorded the emission of the vowel / Ɛ / in ascending and descending glissando, up to the lowest and highest note in the weakest and strongest intensities possible. Results The values of frequencies and respective intensities of the passaggio were identified in ascending and descending, strong and weak emissions in all voice types. There was a higher occurrence of voice break in the high voices, compared to the low ones. The average values ​​of the frequencies found corresponded to different tones from those established in the literature for all voice types. Conclusion The passaggio identified in the vocal range profile of choristers, based on their frequencies and intensities were more frequent in soprano na tenor, compared to alto and bass, in changes to the low and high registers.

ABSTRACT

Resumo Este trabalho aborda a complexa relação humana com o ambiente, preconizando as relações vitais estabelecidas pela própria natureza para subsistir. Para isso, realiza-se pesquisa exploratória sobre bioética ambiental, estruturada a partir de fundamentos filosóficos. Com uma visão sistêmica, procura-se construir e consolidar, de maneira dedutiva, uma ética prática que estimule a impreterível preservação da região amazônica. Delineia-se, então, uma revisão literária narrativa, com destaque para os aspectos histórico-mitológicos da Amazônia, desmatamento e queimadas, e legislação correlacionada e agravos à saúde, ressaltando a importância do diagnóstico interdisciplinar. Como resultado, incita-se a discussão sobre os impactos das atitudes humanas, considerando os vínculos de prevenção e responsabilidade postulados nos valores de bem, justiça e equilíbrio, e tendo consciência de que a harmonia e o cuidar são princípios inerentes à própria existência.


Abstract This work approaches the complex relationship between humans and the environment, preconizing the vital relationships established by the nature itself to subsist. To that end, exploratory research on environmental bioethics, structured from philosophical fundaments, was carried out. With a systemic point of view, it seeks to build and consolidate, deductively, a practical ethic that stimulates the necessary preservation of the Amazonian region. It designs, then, a narrative literary review, focusing on historical-mythological aspects of Amazon, deforestation and fires, and laws regarding health problems, highlighting the importance of interdisciplinary diagnosis. As a result, it urges the discussion on the impacts of human activities, considering links of prevention and responsibility based on values of goodness, justice, and equilibrium, and keeping in mind that harmony and care are principles inherent to existence itself.


Resumen Este trabajo discute la compleja relación entre el ser humano y el medioambiente, preconizando las relaciones vitales establecidas por la propia naturaleza para su persistencia. Para ello, se realiza una investigación exploratoria sobre bioética ambiental a partir de fundamentos filosóficos. Con una mirada sistémica, se propone construir y consolidar deductivamente una ética práctica que impulse la imperativa preservación de la Amazonía. Enseguida, se esboza una revisión literaria narrativa con énfasis en los aspectos histórico-mitológicos de la Amazonía, deforestación e incendios, y la legislación y los problemas de salud correlacionados enfatizando la importancia del diagnóstico interdisciplinario. En los resultados se plantean los impactos de las actitudes humanas teniendo en cuenta los vínculos de prevención y responsabilidad postulados en los valores de bien, justicia y equilibrio, con la concientización de que la armonía y el cuidado son los principios inherentes a la propia existencia.

ABSTRACT Objective The aim of this study was to investigate polycystic ovary syndrome (PCOS) and to explore the relationship between body fat percentage and metabolic markers. Subjects and methods Sedentary women were assigned to PCOS (N = 60) and CONTROL (N = 60) groups. Each group was subdivided into three subgroups according to body fat percentage (22-27%, 27-32% and 32-37%). The protocol consisted of assessments of glucose, insulin, androgens, follicle stimulating hormone (FSH), luteinizing hormone (LH), 17-hydroxyprogesterone (17-OHP), leptin, adiponectin, tumor necrosis factor (TNF-α) and interleukin-6 (IL-6). Results The PCOS subgroups showed higher concentrations of androgens, LH and 17-OHP. Leptin showed direct relationship with increased body fat percentage, whereas adiponectin showed the inverse effect. However, both were unaffected by PCOS. TNF-α and IL-6 were higher in PCOS women and showed a direct relationship with increased body fat percentage. Glucose showed direct relationship with body fat percentage, whereas insulin presented higher values in PCOS women and direct relationship with increased body fat percentage. Conclusions Our findings indicate that PCOS and body fat percentage directly influence concentrations of insulin, TNF-α and IL-6, whereas leptin and adiponectin are influenced only by the increase in body fat percentage in these women. ABSTRACT

Introducción: El síndrome de ALTE es una condición que amenaza la vida de los lactantes; requiere diagnóstico y manejo adecuado para evitar complicaciones o muerte, y sus causas son numerosas, entre ellas: gastrointestinales, respiratorias, neurológicas y otras; las patologías neurológicas representan la tercera causa en la mayoría de estudios. Objetivo: Describir las características de la población de pacientes que consultaron al Hospital de la Misericordia (HOMI) de Bogotá por un episodio de ALTE y que fueron valorados por la especialidad de neuropediatría en el período 2009 a 2013. Materiales y métodos: Se realizó un estudio descriptivo de corte transversal, en una población de 107 pacientes con diagnóstico de ALTE; se determinaron los datos sociodemográficos, características, causas y tratamientos de ALTE; se utilizó el programa SPSS 22. Resultados: La etiología neurológica de ALTE correspondió al 16,8%, correspondiente a crisis epilépticas, trastornos respiratorios de origen central y sangrados de sistema nervioso central. Ocupa el tercer lugar en etiologías, después de las causas gastrointestinales y respiratorias. Conclusión: Es importante definir el diagnóstico y tratamiento de los pacientes con ALTE, pues existe alto riesgo de morbimortalidad asociada. Deben evaluarse factores neurológicos, y no solo gastrointestinales o respiratorios, que ayuden a definir la etiología y evitar complicaciones.


Introduction: ALTE is a condition that threatens the lives of infants; requires diagnosis and appropriate management to avoid complications and / or death. There are numerous causes among which are gastrointestinal, respiratory, and neurological and others. Being the third cause, neurological pathologies in most studies. Objective: To describe the characteristics of the population of patients admitted to Hospital de la Misericord HOMI Bogota, for ALTE and were valued by the specialty of pediatric neurology in the period 2009-2013. Materials and methods: A descriptive cross-sectional study was conducted, with a population of 107 patients with a diagnosis of ALTE; sociodemographic data, characteristics, causes and treatments of ALTE were determined. SPSS 22 software was used. Results: The etiology of neurological ALTE corresponded to 16.8%, corresponding to seizures, respiratory disorders of central origin and CNS bleeds. Ranking as third in etiology below gastrointestinal and respiratory causes. Conclusion: It is important to define the diagnosis and treatment of patients with ALTE, as there is a high risk of morbidity and mortality associated. Factors not only neurological and gastrointestinal or respiratory, to help define the etiology and avoid complications should be evaluated.

Introduction Mercury poisoning causes hearing loss in humans and animals. Acute and long-term exposures produce irreversible peripheral and central auditory system damage, and mercury in its various forms of presentation in the environment is ototoxic. Objective We investigated the otoacoustic emissions responses in a riverside population exposed to environmental mercury by analyzing the inhibitory effect of the medial olivocochlear system (MOCS) on transient otoacoustic emissions (TEOAE). Methods The purpose of the research was to evaluate the entire community independently of variables of sex and age. All of the participants were born and lived in a riverside community. After otolaryngologic evaluation, participants were received tympanometry, evaluation of contralateral acoustic reflexes, pure tone audiometry, and recording of TEOAEs with nonlinear click stimulation. Hair samples were collect to measure mercury levels. Results There was no significant correlation between the inhibitory effect of the MOCS, age, and the level of mercury in the hair. Conclusions The pathophysiological effects of chronic exposure may be subtle and nonspecific and can have a long period of latency; therefore, it will be important to monitor the effects of mercury exposure in the central auditory system of the Amazon population over time. Longitudinal studies should be performed to determine whether the inhibitory effect of the MOCS on otoacoustic emissions can be an evaluation method and diagnostic tool in populations exposed to mercury. .

Introduction: valve disease is an important cause of heart failure. There is a direct relationship between valve deterioration and the patient’s inflammatory status and cytokines: interleukin-6, interleukin-1, tumor necrosis factor, and C-reactive protein, involved in this major state of inflammation. Objective: to report a series of cases of valve replacement, using a bioprosthetic or mechanical valve, and the inflammatory profile of them. Methods: patients older than 18 years and with bioprosthetic or mechanical valve placed for a minimum of 6 months and maximum of 2 years were included. In addition to the demographic characteristics of each patient, inflammatory markers were measured and a comparison was made of echocardiographic results before (based on medical records) and after surgery. A total of 46 patients were enrolled, 23 with mechanical valve and 23 with bioprosthetic valve. Results: of the 46 patients, 20 presented complete data were included, 12 with bioprosthetic and 8 with mechanical valve. There was no difference between types of prosthesis or implant site for the values of inflammatory markers although they were all above reference range. Discussion: patients undergoing aortic mechanical valve implant benefited more than those undergoing bioprosthetic implant and both with much better results than those of valve replacements performed on mitral valve. In short, there was no difference in relation to inflammatory biomarkers. .


Introdução: doença valvar é importante causa de insuficiência cardíaca. Existe relação direta entre a deterioração valvar e o estado inflamatório do paciente, sendo as citocinas interleucina-6, interleucina-1, fator de necrose tumoral e a proteína C reativa as principais envolvidas nesse estado de estimulação. Objetivo: relatar uma série de casos de troca valvar, bioprótese ou mecânica e seu perfil inflamatório. Métodos: pacientes maiores de 18 anos e portadores de bioprótese ou protética mecânica, com período mínimo de 6 meses e máximo de 2 anos, foram incluídos. Além das características demográficas de cada paciente, colheram-se os marcadores inflamatórios e comparou-se o ecocardiograma conforme registro de prontuário antes e depois da cirurgia. Um total de 46 pacientes foi incluído, tendo sido 23 com valva mecânica e 23 de bioprótese. Resultados: dos 46 pacientes, chegamos ao total de 20 pacientes com dados completos, sendo 12 com bioprótese e 8 com protética mecânica. Não houve diferença entre tipo de prótese ou local de implante para os valores dos marcadores inflamatórios, contudo, na média, seus valores estavam aumentados. Discussão: pacientes submetidos ao implante de valva protética mecânica aórtica beneficiaram-se mais do que os submetidos ao implante de bioprótese e ambos com resultado bem superior às trocas realizadas na valva mitral. Não houve diferença em relação aos biomarcadores inflamatórios. .

Objetivo : Estudar a prevalência de zumbido, verificando se há associação entre a queixa desse sintoma e o teor de mercúrio e mensurar seu impacto na qualidade de vida. Métodos : Estudo seccional em toda a população do Lago do Puruzinho, localizada na margem esquerda do Rio Madeira, cidade de Humaitá, Estado do Amazonas, Brasil. Todos os residentes maiores de 18 anos, de ambos os gêneros, foram submetidos à anamnese, avaliação otorrinolaringológica e exame audiológico. Os indivíduos que apresentaram queixa de zumbido responderam à versão traduzida do questionário Tinnitus Handicap Inventory (THI). Também foram pesquisados os teores de mercúrio total no cabelo desses indivíduos. Resultados: Para análise dos resultados sobre a queixa de zumbido, os sujeitos foram divididos em dois grupos quanto à presença ou não de zumbido. O Grupo 1 foi composto por indivíduos que apresentaram queixa de zumbido e o Grupo 2, por indivíduos sem zumbido. Foi observado que 40% dos indivíduos apresentaram escores do THI compatíveis com handicap leve. A análise da associação da presença de zumbido com os teores de mercúrio total no cabelo mostrou que ambos os grupos apresentaram níveis elevados de mercúrio, porém não ocorreram diferenças entre os grupos. Conclusão : Um quarto dos ribeirinhos apresentou queixa de zumbido e exposição significativa ao mercúrio, mas não houve associação entre o zumbido e os níveis elevados de mercúrio. .


Purpose : To study the association between the prevalence of tinnitus and mercury exposure and measure the influence of tinnitus on the quality of life. Methods : We conducted a cross-sectional study of men and women aged above 18 years residing in the Lake Puruzinho region, which is located on the left bank of the Rio Madeira, Humaita city, Amazonas state, Brazil. All subjects underwent anamnesis, otorhinolaryngologic evaluation, and an audiology test. Individuals who experienced tinnitus completed the translated Tinnitus Handicap Inventory (THI). We also examined the levels of total mercury in the hair of these individuals. Results : To analyze the results regarding the prevalence of tinnitus, the subjects were divided into two groups according to the presence or absence of tinnitus. Group 1 was composed of subjects with tinnitus and Group 2 was composed of subjects without tinnitus. Consequently, 40% of individuals had scores consistent with mild handicap according to the THI. The analysis of the association between tinnitus and levels of total mercury in hair samples showed that both groups had high levels of mercury. However, this finding was not significantly different between groups. Conclusion : Herein, 25% of the subjects complained of tinnitus and significant exposure to mercury; however, there was no association between the prevalence of tinnitus and high levels of mercury. .

ABSTRACT

PURPOSE: To analyze the intraoperative and immediate postoperative biochemical parameters of patients submitted to orthotopic liver transplantation. METHODS: Forty four consecutive orthotopic liver transplants performed from October 2009 to December 2010 were analyzed. The patients (38 male and eight female) were divided into two groups: group A, survivors, and group B, non-survivors. Fifty percent of group A patients were Chid-Pugh C, 40% Chid-Pugh B and 10% Chid-Pugh A. In group B, 52% of the patients were Chid-Pugh C, 41% Chid-Pugh B, and 17% Chid-Pugh A. All orthotopic liver transplants were performed by the piggy-back technique without a portacaval shunt in an anhepatic phase. ALT, AST, LDH and lactate levels were determined preoperatively, at five, 60 minutes after arterial revascularization of the graft and 24 and 48 hours after the end of the surgery.( or: after the surgery was finished). RESULTS: There were no preoperative clinical differences (Child and Meld) between the two groups. The times of warm and hypothermal ischemia were similar for both groups (p>0.05). Serum aminotransferases levels at five and 60 minutes after arterial revascularization of the graft were similar (p>0.05) for both groups, as also were lactate levels at the time points studied. There was no significant difference in Δ lactate between groups at any time point studied (p>0.05). No significant difference was observed between groups during the first 24 and 48 hours after surgery (p>0.05). CONCLUSION: No significant difference in any of the parameters studied was observed between groups. Under the conditions of the present study and considering the parameters evaluated, no direct relationship was detected between the intraoperative situation and the type of evolution of the patients of the two groups studied.

ABSTRACT

PURPOSE: The objective of the present study was to evaluate the postoperative levels of classical or pure MELD and changes in lactate or base excess (BE) levels as possible predictive factors of the type of outcome of patients submitted to orthotopic liver trasplantation (OLT). METHODS: The study was conducted on 60 patients submitted to OLT at the University Hospital, Faculty of Medicine of Ribeirão Preto, USP, between October 2008 and March 2012. The 30 latest survivor (S) and non-survivor (NS) cases were selected. All liver transplants were performed using the piggy-back technique. ALT, AST, BE and blood lactate values were determined for each group at five time points (immediate preoperative period, end of hypothermal ischemia, 5 and 60 minutes after arterial revascularization and in the immediate postoperative period, when the postoperative MELD was also calculated. RESULTS: The aminotransferases reached a maximum increase 24 hours after surgery in both the S and NS groups. There was a significantly higher increase in BE and blood lactate in the NS group, especially after 5 minutes of afterial reperfusion of the graft, p<0.05. There was no significant difference in preoperative MELD between groups (p>0.05), while the postoperative MELD was higher in the NS than in the S group (p<0.05) CONCLUSION: Joint analysis of postoperative MELD, BE and blood lactate can be used as an index of severity of the postoperative course of patients submitted to liver transplantation.


OBJETIVO: O objetivo do presente estudo é avaliar os níveis do MELD clássico ou puro pós-operatório, alterações dos níveis séricos do lactado e BE, analisando-os como possíveis fatores preditivos do tipo de sobrevida de pacientes submetidos ao transplante ortotópico de fígado (OLT). MÉTODOS: Foram analisados 60 pacientes submetidos ao OLT (técnica de piggy-back) no Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto-USP, entre Outubro 2008 e Março de 2012. Foram selecionados os 30 últimos casos de sobreviventes (S) e 30 de não-sobreviventes (NS). Avaliou-se para cada grupo, os valores ALT, AST, de base excess (BE) e lactato sanguíneos em cinco momentos (pré-operatório imediato, no final da isquemia hipotérmica, 5 e 60 minutos após a revascularização arterial e no pós-operatório imediato, quando também foi calculado o MELD pós-operatório. RESULTADOS: Com relação às aminotransferases, houve um aumento máximo após 24 horas de pós-operatório em ambos os grupos S e NS. Houve aumento significativo dos níveis de BE e lactato sanguíneo significativamente maior no grupo NS, sobretudo nos tempos após 5 minutos de reperfusão arterial do enxerto, p<0,05. Não houve diferença significativa entre o MELD pré-operatório em ambos os grupos (p>0,05). O MELD pós-operatório foi maior no grupo NS do que no grupo S (p<0,05). CONCLUSÃO: A análise conjunta do MELD pós-operatório, do BE e do lactato sanguíneo pode ser usada como índice de gravidade da evolução pós-operatória de pacientes submetidos ao OLT.

ABSTRACT

O acidente vascular encefálico é a principal causa de incapacidade crônica em adultos, porém poucos estudos avaliaram a motricidade do tronco nestes indivíduos. OBJETIVO: Comparar a atividade mioelétrica do tronco entre indivíduos hemiparéticos e controle. MÉTODO: Foram avaliados oito indivíduos hemiparéticos e oito controles, por meio de eletromiografia de superfície (EMGs), durante atividades de flexão dos membros inferiores e rotação do tronco em supino; levantar e elevação dos membros superiores na posição sentada. RESULTADOS: O músculo reto abdominal parético apresentou maior ativação que o grupo controle (p=0,031) durante a flexão dos membros inferiores. Os músculos oblíquos apresentaram, bilateralmente, maior ativação na elevação dos membros inferiores do que na rotação (p=0,014 e p=0,002, respectivamente). Não houve diferenças entre músculos eretores durante as atividades. CONCLUSÃO: Os músculos flexores do tronco de indivíduos hemiparéticos apresentaram alterações motoras no reto abdominal do lado parético e realizam compensações por meio dos oblíquos externos.


Stroke is the main cause of chronic disability in adults, but few studies evaluated the trunk motor activity in affected subjects. OBJECTIVE: To compare the myoelectrical activity of trunk muscle in hemiparetic and control subjects. METHOD: Eight hemiparetic and eight control subjects were assessed during lower extremities flexion and trunk rotation in supine position; stand up and rise up upper extremities in seated position. RESULTS: Paretic rectus abdominis presented a higher activation than control group (p=0.031) during lower extremities elevation. Obliquus externus abdominis showed a bilateral higher activation during lower extremities elevation than rotation activities (p=0.014 and p=0.002, respectively). There was no difference in extension activities comparison. CONCLUSION: Trunk flexor muscles of hemiparetic subjects demonstrated motor alterations in rectus abdominis of paretic side and compensations were used with Obliquus externus.
